About the Role

The Physical Therapist conducts individualized therapy programs, including assessment, treatment, planning, and education, to optimize patient progress toward functional goals. They evaluate patient needs, develop and adjust care plans, and collaborate with healthcare professionals. The role includes supervising support staff, ensuring compliance with standards, maintaining documentation, and promoting professional growth through ongoing education. The PT also provides leadership, acts as a community resource, and upholds safety, infection control, and resident confidentiality policies.

Responsibilities

• Conduct individualized therapy programs, including assessment, treatment, planning, and education
• Evaluate patient needs and develop/adjust care plans
• Collaborate with healthcare professionals to optimize patient progress
• Supervise support staff and ensure compliance with standards
• Maintain accurate documentation
• Promote professional growth through ongoing education
• Provide leadership and act as a community resource
• Uphold safety, infection control, and resident confidentiality policies

Requirements

• Graduate of an APTA-approved program
• Current state license from the Board of Physical Therapy Examiners
• At least one year of experience as a physical therapy aide
