Physical Therapist (PT)

About the role

The Physical Therapist provides direct rehabilitation care in a skilled nursing facility setting, focusing on long-term and short-term recovery for a diverse patient population with orthopedic, neurological, and cardiopulmonary needs.

Responsibilities

• Develop, assess, and modify individualized treatment plans to improve patient function and quality of life.
• Supervise and evaluate therapy staff, including PTAs, rehab aides, and students.
• Ensure proper documentation, utilize therapy software, and maintain patient safety and rights.
• Collaborate with interdisciplinary teams and communicate effectively with patients and families.
• Participate in community health initiatives and uphold professional standards and confidentiality.

Qualifications

• Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT).
• Active state licensure in good standing.
• Preferred one year of post-acute care experience.
• CPR certification (obtained upon hire or during orientation).
• Proficiency in Microsoft Word, Excel, and email.
• Strong communication, judgment, and teamwork skills.
• Knowledge of laws, regulations, and procedures relevant to post-acute care.

• Chart accurately and timely, ensuring compliance and confidentiality.
• Demonstrate professionalism, integrity, and excellent customer service.
• Assist in patient evacuation and maintain dependable attendance.
• Participate in team activities, communicate effectively, and maintain good body mechanics during work.
