About the role
The Physical Therapist provides direct rehabilitation care in a skilled nursing facility setting, focusing on long-term and short-term recovery for a diverse patient population with orthopedic, neurological, and cardiopulmonary needs.
Responsibilities
- Develop, assess, and modify individualized treatment plans to improve patient function and quality of life.
- Supervise and evaluate therapy staff, including PTAs, rehab aides, and students.
- Ensure proper documentation, utilize therapy software, and maintain patient safety and rights.
- Collaborate with interdisciplinary teams and communicate effectively with patients and families.
- Participate in community health initiatives and uphold professional standards and confidentiality.
Qualifications
- Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT).
- Active state licensure in good standing.
- Preferred one year of post-acute care experience.
- CPR certification (obtained upon hire or during orientation).
- Proficiency in Microsoft Word, Excel, and email.
- Strong communication, judgment, and teamwork skills.
- Knowledge of laws, regulations, and procedures relevant to post-acute care.
Key Duties
- Follow established treatment plans and safety protocols.
- Set measurable, functional goals for patient progress.
- Chart accurately and timely, ensuring compliance and confidentiality.
- Demonstrate professionalism, integrity, and excellent customer service.
- Assist in patient evacuation and maintain dependable attendance.
- Participate in team activities, communicate effectively, and maintain good body mechanics during work.
