About the role

Care and compassion are at the heart of this home-based Physical Therapist role. You will join an interdisciplinary team delivering compassionate, quality care and positive outcomes for patients. Duties include providing skilled therapy in patients’ homes as directed by the attending physician, educating patients and families, and ensuring patient safety. Home care offers real-world context that helps you tailor interventions to restore health and prevent re-hospitalizations.

Responsibilities

  • Patient Care & Treatment: Provide skilled physical therapy in the patient’s home as ordered by the physician; relieve pain, restore or maintain function using exercise, massage, heat, water, etc.; observe and document patient reactions, changes, and progress; front-load visits for high-risk patients and teach preventive measures; arrange for outpatient services when needed.
  • Planning & Assessment: Perform OASIS assessments to develop individualized plans of care; complete initial evaluation and admission visit within 48–72 hours of referral; evaluate and adjust the plan of care based on patient response.
  • Family & Team Communication: Counsel and instruct patients and families on disease/injury management and equipment use (wheelchairs, walkers, etc.); ensure understanding of financial responsibility for equipment via DME department/MSW; oversee and supervise therapy services provided by PTAs and nurse assistants; communicate with schedulers, PTAs, and nurse aides regarding results and changes.
  • Compliance & Administration: Complete all clinical documentation per agency protocol and Medicare/Federal guidelines; document treatment, assessments, and patient education; participate in on-call requirements, case conferences, and mandatory meetings; maintain professional appearance and agency policies.

Requirements

  • Physical Therapist with current license in the state of employment.
  • Minimum one year of experience as a PT in an acute care setting; home care experience preferred.
  • Valid driver’s license and auto insurance in your name.
  • Capable of meeting all physical demands of the role.
