Physical Therapist (PT)

Key Responsibilities

• Develop, implement, and modify individualized treatment plans based on physician referrals.
• Supervise and evaluate care provided by PTAs, rehab aides, and students.
• Maintain accurate and timely documentation of patient progress and therapy activities.
• Collaborate with an interdisciplinary team to ensure comprehensive patient care.
• Assist in patient transfers and emergency evacuations, ensuring safety and comfort.
• Provide excellent patient service with a positive attitude, demonstrating professionalism and integrity.
• Participate in community health initiatives and adhere to all regulatory standards and facility policies.

Qualifications & Skills

• Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT).
• Current state licensure or registration in good standing.
• CPR certification required and must be maintained.
• Preferred one year of experience; new graduates are welcome.
• Proficient in Microsoft Word, Excel, and email communication.
• Strong knowledge of post-acute care practices, laws, and safety procedures.
• Excellent communication, decision-making, and teamwork skills.
• Physically capable of performing tasks involving movement, lifting, and positioning.
