About the role

The Physical Therapist (PT) role at Adaptive Home Health is a core part of our mission to dramatically improve patient access to home health care. You restore function, keep patients safe at home, and drive real outcomes.

Responsibilities

  • Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.
  • Complete comprehensive in-home assessments and deliver skilled PT visits.
  • Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
  • Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
  • Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
  • Manage treatment and outcomes directly, including PTA supervision where appropriate.
  • Coach patients and caregivers with practical home programs that actually get done.
  • Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

Requirements

  • Must-have: Active Texas PT license (in good standing), strong clinical judgment and comfort working independently in the field, comfortable escalating changes in condition to RN/MD and documenting rationale clearly, strong communication and documentation, reliable follow-through on orders, frequencies, and patient goals, current driver’s license and comfort with daily travel across assigned territory, able to carry/transport basic therapy equipment as needed, competency in core home health PT scope, safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations, strengthening, functional mobility, and home safety assessment/modification education, patient/caregiver coaching with practical home programs.
  • Nice-to-have: Prior home health experience, Homecare Homebase (HCHB) experience (or similar home health EMR), deeper clinical experience in high-signal PT populations, experience supervising/mentoring PTAs and collaborating on care plans, strong organization and time management.

Pay

Compensation (Productivity Points): 75 dollars per point

Schedule

Schedule: Full-time Monday–Friday + occasional weekends (as needed)
