About the role

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care. The PT role is a core part of that mission: you restore function, keep patients safe at home, and drive real outcomes. We also build technology to support field clinicians — our charting product reduces documentation burden, and our scheduling product (launching soon) will make routing and visit coordination easier, protecting your time.

Responsibilities

  • Own the caseload as a case manager: accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.
  • Complete comprehensive in-home assessments and deliver skilled PT visits.
  • Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
  • Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
  • Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
  • Manage treatment and outcomes directly, including PTA supervision where appropriate.
  • Coach patients and caregivers with practical home programs that actually get done.

Requirements

  • Active Texas PT license in good standing.
  • Strong clinical judgment and comfort working independently in the field.
  • Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
  • Strong communication and documentation skills.
  • Reliable follow-through on orders, frequencies, and patient goals.
  • Current driver’s license and comfort with daily travel across assigned territory.
  • Able to carry/transport basic therapy equipment as needed.
  • Competency in core home health PT scope:
    • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations.
    • Strengthening, functional mobility, and home safety assessment/modification education.
    • Patient/caregiver coaching with practical home programs.

Qualifications

  • Prior home health experience.
  • Homecare Homebase (HCHB) experience (or similar home health EMR).
  • Deeper clinical experience in high-signal PT populations.
  • Experience supervising/mentoring PTAs and collaborating on care plans.
  • Strong organization and time management.

Pay

$75 per productivity point; target of 28–30 points per week.

Schedule

Full-time, Monday–Friday with occasional weekends as needed.
