Physical Therapist (PT)

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model. Our ultimate mission is to dramatically improve patient access to home health care. As a Physical Therapist, you’ll deliver safe, reliable skilled care in the home, with excellent communication and escalation to the RN/clinical team to ensure patients stay safe and on track.

- Smarter charting: Record or dictate visits with AI Scribe, start with prefilled assessments, extract medication lists from photos, and keep all visit information in one easy-to-use place.

The result: less administrative burden, fewer coordination headaches, and more time for patient care.
Responsibilities
- Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.
- Complete comprehensive in-home assessments and deliver skilled PT visits.
- Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
- Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
- Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
- Manage treatment and outcomes directly, including PTA supervision where appropriate.
- Coach patients and caregivers with practical home programs that actually get done.
- Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
Requirements
Must-haves
- Active Texas PT license (in good standing).
- Strong clinical judgment and comfort working independently in the field.
- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
- Strong communication and documentation.
- Reliable follow-through on orders, frequencies, and patient goals.
- Current driver’s license and comfort with daily travel across assigned territory.
- Able to carry/transport basic therapy equipment as needed.
- Competency in core home health PT scope.
- Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations.
- Strengthening, functional mobility, and home safety assessment/modification education.
- Patient/caregiver coaching with practical home programs.
Nice-to-haves
- Prior home health experience.
- Homecare Homebase (HCHB) experience (or similar home health EMR).
- Deeper clinical experience in high-signal PT populations.
- Experience supervising/mentoring PTAs and collaborating on care plans.
- Strong organization and time management.
Schedule
Full-time Monday–Friday + occasional weekends (as needed).
Pay
$75.00 per productivity point, with an expected range of $100,000 - $130,000 per year.

As a PRN Physical Therapist, you’ll restore function, keep patients safe at home, and drive real outcomes. We also build technology to support field clinicians, reducing documentation burden and making routing and visit coordination easier.

- Deliver skilled PT visits and evaluations based on patient need and your availability.
- Complete comprehensive in-home assessments and deliver skilled PT visits.

Part-time flexibility: Pick up visits based on your availability, including weekdays and/or weekends as needed.
Pay
Compensation is based on productivity points, with a full-time benchmark of 30 points/week. You can take on more or fewer visits based on your availability.

We also build technology to support field clinicians, reducing documentation burden and making routing and visit coordination easier.

- Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
- Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.
- Drive multidisciplinary coordination: escalate changes in condition, align on orders, and keep the care team moving.
- Coach patients and caregivers with practical home programs that actually get done.
- Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
