About the Role
The Physical Therapist provides direct rehabilitation care to patients in a skilled nursing environment, focusing on improving functional abilities and quality of life.
Responsibilities
- Develop and implement personalized treatment plans
- Monitor patient progress and document care accurately
- Oversee therapy team members such as PTAs and rehab aides
- Collaborate with interdisciplinary teams to ensure patient safety
- Adhere to legal and organizational standards
- Assist with patient transfers and provide excellent customer service
- Participate in community health initiatives
- Follow treatment plans and assess/modify goals as needed
- Evaluate care given by therapy staff
- Maintain timely documentation
- Demonstrate dependable attendance
- Maintain confidentiality and adhere to policies
- Foster a positive, patient-centered environment
Qualifications & Skills
- Graduate of an accredited physical therapy program
- Licensed in the state
- CPR certified
- New graduates are encouraged to apply
- Proficiency in Microsoft Office
- Good body mechanics
- Strong communication skills and professionalism
- Ability to make independent decisions and work collaboratively
