Physical Therapist (PT)

Role Overview

The Physical Therapist (PT) provides direct rehabilitation care in physical therapy for patients in a skilled nursing facility setting.

Responsibilities

• Following individualized treatment plans and establishing measurable goals.
• Overseeing and evaluating care provided by PTAs, rehab aides, and students.
• Charting accurately and utilizing therapy software efficiently.
• Collaborating with an interdisciplinary team to promote patient progress.
• Assisting in patient evacuation and ensuring safety protocols.
• Maintaining professionalism, confidentiality, and adherence to policies.
• Participating in community health initiatives when appropriate.

Qualifications & Skills

• Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT).
• Current licensure in applicable state and CPR certification.
• Preferred experience in post-acute care.
• Proficiency in Microsoft Office and excellent communication skills.
• Ability to make independent decisions and demonstrate good body mechanics.
• Commitment to patient-centered care with a positive attitude and dependable attendance.
