Role Overview
The Physical Therapist (PT) provides direct rehabilitation care in physical therapy for patients in a skilled nursing facility setting.
Responsibilities
- Following individualized treatment plans and establishing measurable goals.
- Overseeing and evaluating care provided by PTAs, rehab aides, and students.
- Charting accurately and utilizing therapy software efficiently.
- Collaborating with an interdisciplinary team to promote patient progress.
- Assisting in patient evacuation and ensuring safety protocols.
- Maintaining professionalism, confidentiality, and adherence to policies.
- Participating in community health initiatives when appropriate.
Qualifications & Skills
- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT).
- Current licensure in applicable state and CPR certification.
- Preferred experience in post-acute care.
- Proficiency in Microsoft Office and excellent communication skills.
- Ability to make independent decisions and demonstrate good body mechanics.
- Commitment to patient-centered care with a positive attitude and dependable attendance.
