About the Role
The Physical Therapist provides evaluation and treatment to patients, functioning as a member of the interdisciplinary team. Responsibilities include patient and family education, planning and modifying treatment procedures, maintaining documentation, managing a caseload, and attending patient conferences and agency in-services.
Responsibilities
- Provides patient care and documents treatment in accordance with physician orders, agency policy, and patient needs.
- Performs patient evaluations and selects appropriate diagnostic material to assess all aspects of patient care.
- Informs the physician of assessment results and recommended clinical interventions prior to initiating care.
- Develops and documents appropriate treatment plans, including intervention approaches, strategies, materials, and treatment priorities.
- Provides organized and efficient patient treatment, including advising, educating, and counseling the patient.
- Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
- Provides appropriate treatment within the parameters set by the agency and payer.
- Determines objective and measurable goals in collaboration with team members and patients.
- Meets agency productivity standards and uses work hours efficiently.
Requirements
- Master’s degree in physical therapy from a CAPTE-accredited program.
- Minimum nine months of experience as a physical therapist.
- Preferred community/home health experience.
- Currently licensed to practice as a physical therapist within the state of operation.
- Must possess and maintain valid CPR certification while employed in a clinical role.
- Must be a licensed driver who can travel to all business locations.

Pay
$95,000.00 - $105,000.00 per year (paid on a pay-per-point basis). The compensation reflected is an estimate of annual compensation for full-time status.
Schedule
PRN, Monday through Friday, 8:00 AM - 5:00 PM.
Coverage area: St. Paul and surrounding areas.
