About the Role

The Physical Therapist provides evaluation and treatment to patients, functioning as a member of the interdisciplinary team. Responsibilities include patient and family education, planning and modifying treatment procedures, maintaining documentation, managing a caseload, and attending patient conferences and agency in-services.

Responsibilities

  • Provides patient care and documents treatment in accordance with physician orders, agency policy, and patient needs.
  • Performs patient evaluations and selects appropriate diagnostic material to assess all aspects of patient care.
  • Informs the physician of assessment results and recommended clinical interventions prior to initiating care.
  • Develops and documents appropriate treatment plans, including intervention approaches, strategies, materials, and treatment priorities.
  • Provides organized and efficient patient treatment, including advising, educating, and counseling the patient.
  • Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
  • Provides appropriate treatment within the parameters set by the agency and payer.
  • Determines objective and measurable goals in collaboration with team members and patients.
  • Meets agency productivity standards and uses work hours efficiently.

Requirements

  • Master’s degree in physical therapy from a CAPTE-accredited program.
  • Minimum nine months of experience as a physical therapist.
  • Preferred community/home health experience.
  • Currently licensed to practice as a physical therapist within the state of operation.
  • Must possess and maintain valid CPR certification while employed in a clinical role.
  • Must be a licensed driver who can travel to all business locations.

Pay

$95,000.00 - $105,000.00 per year (paid on a pay-per-point basis). The compensation reflected is an estimate of annual compensation for full-time status.

Schedule

PRN, Monday through Friday, 8:00 AM - 5:00 PM.

Coverage area: St. Paul and surrounding areas.
