Role Overview

A Home Health Physical Therapist provides personalized physical therapy services to patients in their homes, promoting functional improvement, mobility, and independence.

Key Responsibilities

• Planning and administering treatment
• Assessing patient strengths
• Developing rehab programs aligned with physician plans
• Supervising PTAs and aides
• Documenting patient progress
• Communicating updates to the care team, patients, and families

Qualifications & Skills

• Current, unrestricted Physical Therapy license
• CPR certification

Additional Details

• Patient interaction required
• TB screening required
• Valid driver's license with insurance required
• Collaboration with healthcare teams preferred
• Prior experience as a PT preferred

Pay

• $71–$99 per visit/unit
• $111,600–$153,600 annually
