Accomac, Virginia – Hiring Range $55.00 – $63.00/Hourly. Actual pay is determined by program, work commitment and business needs.

Overview

The Physical Therapist will follow appropriate practices, to perform evaluations, timely re-evaluations and OASIS assessments, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. This position must also be able to follow and adhere to the practices described in the Department Policy and Procedure Manual. The Therapist is required to be flexible with daily scheduling and may be required to work various patient care settings.

Responsibilities

  • Perform evaluations, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability.
  • Evaluate and assess patients timely, based on practice guidelines, evidence‑based guidelines and regulatory standards (state & organizational policies, accrediting bodies requirements, etc.).
  • Provide reassessments including OASIS assessments at appropriate time points to determine level of functioning.
  • Update and revise the plan of care in consultation with the physician and interdisciplinary team members.
  • Build trusting collaborative relationships with a multi‑disciplinary team to support patient care needs.
  • Actively participate in multi‑disciplinary team meetings, as assigned.
  • Supervise Physical Therapy Assistants and Home Health Aides in providing services, including completion of required supervisory visit documentation.
  • Participate in training and support of all team members, volunteers and students.
  • Provide therapeutic treatments and evaluate equipment needs as applicable.
  • Prepare clinical and progress notes that support skilled intervention and assess outcomes of care.
  • Plan for discharge of patient.
  • Carry out treatment plan effectively and modify it to maximize therapeutic benefit, meet discharge criteria and return to optimal level of care.
  • Utilize testing results and develop updated plan of care based on evidence‑based guidelines.

Qualifications

  • Education: Bachelor’s Degree in Physical Therapy (required); Master’s Degree in Physical Therapy (required); Doctoral Degree in Physical Therapy (required).
  • Experience: Minimum 1 year of Physical Therapy experience (required).
  • Skills and Abilities: Knowledge of growth and development principles across the lifespan; ability to assess patient needs, analyze evaluated data, interpret information to identify age‑specific needs and provide appropriate care; demonstrate knowledge and skills to provide age, goal, and diagnosis‑appropriate care.
  • Licenses and Certifications: Physical Therapy (PT) license – Virginia Department of Health Professions (VDHP) (required upon hire); CPR/BLS Certification – American Heart Association/American Red Cross/American Safety and Health Institute (required within 30 days); Valid driver’s license; personal vehicle required for RHS business 25 % or more of the time.

Pay

Hiring Range: $55.00 – $63.00 per hour. Actual pay is determined by program, work commitment and business needs.
