What You’ll Do

• Provide individualized physical therapy evaluations and treatment plans based on each resident’s goals and needs
• Build meaningful relationships with residents, families, nursing staff, and interdisciplinary team members
• Collaborate closely with the care team to support the best possible outcomes
• Document patient progress using Net Health and facility systems
• Contribute to a positive, team-focused therapy culture where support, ideas, and encouragement are shared

What You’ll Bring

• Degree from an accredited Physical Therapy program
• Active Maryland Physical Therapist license in good standing
• A genuine passion for working with older adults
• Strong communication skills and a collaborative mindset
