Physical Therapist (PRN)

About the role

We are seeking a PRN Physical Therapist to join our Therapy Services team.

Responsibilities

• Screen, evaluate, re-evaluate, discharge plan and make revisions in any established plan of care as supervised by the physician
• Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual
• Re-assess patients and caseload as needed in order to determine need for continued services or discontinuation of services based on patient progress
• Carry out treatment plan as indicated
• Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated
• Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment
• Educate staff, family, and care givers as indicated
• Identify needs for program development and implement programs according to company policies and procedures
• Identify equipment needs of Physical Therapy Dept. and order equipment according to company and facility policy
• Complete all required documentation according to guidelines specified in Documentation Manual
• Coordinate in-services for facility staff as need is determined, to ensure safe and effective carry over of Maintenance/Restorative Programs and overall call of the residents

Requirements

• Degree in Physical Therapy from an accredited program
• Active state license required
• Current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines
• Ability to supervise PTAs and PT Techs
