Physical Therapist (PRN)

About the role

Encore Village is seeking a PRN Physical Therapist for our outpatient departments. The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. They identify the need for and make referrals to other disciplines to address the comprehensive needs of patients, implement individualized therapy programs designed to restore, reinforce, and enhance performance, and instruct, educate, and train patients and caregivers in skills and functions essential for promoting independence and productivity, to diminish or correct pathology. The role also involves directing activities to promote and maintain health.

Qualifications

• Graduate of an accredited Physical Therapist (PT) therapy program
• Current Illinois State Licensure
• CPR Certified
• Proficient with therapy documentation (NetHealth)
