Physical Therapist - Per Diem Weekends

About the role

We are seeking a compassionate Home Health Physical Therapist (PT) for a Per Diem WEEKEND role, providing clinical care directly in patients' homes in Lebanon, MO, and surrounding areas.

Responsibilities

• Make home visits to assess and treat clients recovering from injuries, illnesses, surgeries, or hospitalizations.
• Perform diagnostic tests such as mobility, strength, balance, and safety assessments.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and revise individualized care plans, including manual therapy, gait training, and balance exercises.
• Collaborate with the multidisciplinary team and instruct clients and families in rehabilitative activities.
• Document observations and interventions accurately using digital tools.

Requirements

• Active and current PT license.
• Ability to travel locally to client locations.

Qualifications

• Preferred: one year of clinical experience; home health experience is a plus but not required—training provided.
