About the Role
Provide in-home clinical care to patients in designated areas as a Home Health Physical Therapist (PT).
Responsibilities
- Conduct home visits for patients recovering from injury, illness, or surgery
- Perform diagnostic tests and assessments
- Develop and revise personalized care plans
- Implement manual therapies, gait training, and balance interventions
- Educate clients and families on rehabilitative activities
- Document care using digital tools
- Collaborate with interdisciplinary teams
Requirements
- Active Physical Therapist (PT) license
- Preferred one year of clinical experience (training available for those new to home health)
- Ability to travel locally
- Compassionate, reliable, and attentive to patient needs
