Fairport, NY 14450 – Clinical Therapy Department. Per diem substitute for the 2026-2027 school year; start date as needs present.

Pay

$40.00/hour. Actual salary is dependent upon education, experience, and contract.

Responsibilities

  • Evaluate, plan programs, and provide physical therapy services for students with disabilities, ages 5 to 21.
  • Develop and implement IEP and annual goals/objectives based on each student’s needs or 504 accommodation plans.
  • Deliver ongoing physical therapy services within program assignment in collaboration with teachers, other school professionals, parents, and appropriate community agencies/services.
  • Participate on school/building/program resource teams and in staffing; interact with Committees on Special Education.
  • Provide thorough and timely reporting consistent with BOCES and departmental procedures and procedures of the program or district assignment.
  • Implement various service delivery models: individual, small group, integrative classroom, RtI, etc.
  • Establish and maintain a daily schedule.
  • Establish and maintain professional working relationships with school professionals, parents, students, and fellow colleagues within the OT/PT Department.
  • Attend departmental and program assignment meetings.
  • Prepare CSE packets as required by assignment.
  • Comply with BOCES policies.
  • Provide staff development for the OT/PT Department and/or clinical education of PT students as needed.
  • Supervise Physical Therapist Assistants.
  • Maintain licensure through ongoing professional education.
  • Complete daily documentation that complies with NYSED and Medicaid requirements.

Requirements

  • Civil Service eligibility as a Physical Therapist:
    • Possession of a valid license and currently registered by the New York State Education Department as a Physical Therapist; OR
    • Possession of a Limited Permit to practice Physical Therapy issued by the New York State Education Department.
  • Master’s or Doctorate Degree in Physical Therapy from an accredited program.
  • Experience with Special and General Education populations preferred.
  • Experience and/or training within sensory integration and neurodevelopment approaches preferred.
  • Experience and/or knowledge of adaptive equipment.
  • Knowledge of or willingness to be trained in NY Medicaid Guidelines.
  • National Provider Number (NPI) or willingness to obtain one.
  • Physically able to perform essential functions of the position (with or without reasonable accommodation).
