$60–$65/hour BOE

Responsibilities
- Evaluate residents and determine physical therapy needs and rehabilitation potential
- Develop, implement, and modify individualized treatment plans
- Provide treatment focused on gait training, transfers, balance, strengthening, mobility, and pain management
- Communicate with physicians, nursing staff, residents, and families regarding progress and goals
- Participate in care plan meetings and interdisciplinary team conferences
- Complete timely, accurate, and compliant documentation
- Educate residents and caregivers on exercises, mobility safety, and fall prevention
- Recommend assistive devices and durable medical equipment such as walkers, canes, and wheelchairs
- Supervise PTAs and therapy aides as appropriate
- Maintain a safe treatment environment and follow infection control procedures
Qualifications
- Doctorate or Master’s Degree in Physical Therapy from an accredited program required
- Current California Physical Therapist license in good standing
Preferred Skills
- Experience with Casamba and PointClickCare
- Strong communication and interpersonal skills
- Excellent clinical judgment and problem-solving abilities
- Passion for patient-centered care and functional outcomes
