Physical Therapist Per Diem
About the Role

We are dedicated to helping people feel their best, fostering meaningful connections with patients, families, and communities. This per diem role requires a minimum of 30 hours per month.
Responsibilities
- Complete assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Develop and implement a treatment plan based on patient assessments and evaluations.
- Provide services within the scope of practice as defined by state laws, in accordance with the plan of care and in coordination with the healthcare team.
- Observe and report the patient’s response to treatment and any changes in condition to the supervising nurse and/or physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
- Instruct and inform the healthcare team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Visit patients according to the Plan of Treatment, complete progress notes for each visit, and submit them per policy.
- Complete all patient evaluations and develop the physical therapy plan of care within state-specific guidelines.
- Report outcomes of evaluations, goals, and anticipated projected frequency of care.
- Function in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
Requirements
- Current Physical Therapy licensure in the state of practice.
- Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
Preferred Qualifications
- Current CPR certification or ability to complete within 90 days of hire.
Pay
Annual total cash target pay range: $104,840 - $157,259.
- Hourly rate: $50.40 - $75.61.
- Per visit point rate: $60.48 - $90.73.
Annual total cash compensation assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including local labor markets, education, work experience, and may increase over time based on productivity and performance in the role.
This role receives two types of compensation depending on the work being performed:
- Per visit point rate compensation for visits, calculated by multiplying the per visit point rate by the productivity points accrued for various types of visits. Each visit type is assigned a certain number of productivity points inclusive of direct and indirect patient care activities.
- Hourly rate for non-visit activities such as orientation.
