Physical Therapist (Per Diem)

Responsibilities

• Direct patient care and provide physical treatment to adult patients.
• Establish short and long-term rehabilitation goals based on an individualized patient treatment plan.
• Measure and document progress towards these goals.
• Instruct patients and caregivers in activities to be performed between treatments and upon discharge.
• Coordinate and communicate the patient's plan of care and findings with Rehabilitation Supervisors, physicians, and other healthcare disciplines as necessary.

Requirements

• Minimum Bachelor of Science in Physical Therapy from an accredited institution.
• Minimum of one year of experience providing therapy treatments in a healthcare setting.
• Registered and licensed Physical Therapist in New York State.
• Valid CPR certification.
