Physical Therapist - Per Diem

About the role

Provide home health physical therapy services, including patient assessments, developing individualized treatment plans, and performing interventions to improve mobility, function, and pain management.

Evaluate patients' needs, collaborate with physicians and the healthcare team, observe treatment responses, and educate patients and families on care plans.

Deliver equitable, high-quality care in diverse home settings and foster a culture of caring and professional growth.

Responsibilities

• Complete comprehensive patient evaluations
• Develop and implement treatment plans
• Document progress and ensure treatment aligns with state regulations
• Collaborate with physicians and healthcare team members
• Educate patients and families on care plans

Requirements

• Current Physical Therapy license
• Valid driver’s license

CPR certification preferred.
