About the role
This position involves evaluating and treating patients in home care settings to restore function, relieve pain, and improve movement, under physician orders. The Physical Therapist collaborates with healthcare teams to provide compassionate, patient-centered care aligned with their State Practice Act.
Responsibilities
- Assess patient needs and develop comprehensive care plans.
- Apply various therapeutic procedures to promote recovery and mobility.
- Work as part of an integrated healthcare team to ensure quality care.
- Use office equipment such as tablets and cell phones for documentation and communication.
- Travel within the service area to patient homes, demonstrating professional procedures in a home setting.
Qualifications
- Graduate of an APTA-approved college of Physical Therapy.
- Current state license and valid driver’s license.
- Excellent communication and interpersonal skills.
- CPR certification required.
- Ability to assess patient needs, develop, and implement treatment plans.
- Flexible schedule including weekends and holidays.
- Ability to work independently and demonstrate professional procedures in home environments.
