About the role
We are seeking a Physical Therapist to provide outpatient rehabilitation services in in-home settings, primarily for older adults.
Responsibilities
- Evaluating patients
- Developing and implementing personalized therapy programs
- Instructing patients and caregivers to promote independence
- Supervising support personnel
- Ensuring timely documentation
- Making multidisciplinary referrals
- Maintaining a safe environment
- Performing assessments
