Physical Therapist - Outpatient Therapy

About the role

We are seeking a Licensed Physical Therapist to evaluate and treat patients of all ages, developing and adjusting personalized care plans based on patient responses and evidence-based practices.

Responsibilities

• Performing patient assessments, examinations, and objective testing for impairments related to orthopedic and neurologic conditions.
• Designing and implementing treatment plans with goals, outcomes, and discharge strategies.
• Educating patients, families, caregivers, and healthcare team members using effective teaching methods.
• Utilizing therapeutic modalities, applying tape, and supporting patient mobility and stabilization.
• Documenting care accurately in the electronic medical record and communicating patient progress to providers.
• Supervising support personnel and ensuring compliance with legal and ethical standards.
• Completing required continuing education to maintain licensure and meet program standards.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) in Illinois.
• Basic Life Support (BLS) certification.
