Physical Therapist - Outpatient Therapy

About the Role

Provides comprehensive physical therapy services to patients of all ages, evaluating and treating based on individual needs. Acts as a clinical resource and role model for staff and students, ensuring safe patient transfers, proper body mechanics, and respectful, empathetic care.

Responsibilities

• Performs patient assessments and develops personalized care plans.
• Adjusts treatment plans based on clinical findings and patient progress.
• Documents evaluations, interventions, and outcomes in accordance with organizational standards.
• Utilizes standardized outcome measures to track patient progress.
• Educates patients and families on treatment plans, safety, and rehabilitation processes.
• Supervises support personnel and communicates patient progress and discharge plans effectively.
• Collaborates with the healthcare team throughout the rehabilitation process.
• Ensures quality care with a focus on safety, dignity, and functional activities.

Requirements

• Bachelor's Degree in Physical Therapy.
• Licensed as a Physical Therapist (PT) in Illinois.
• Basic Life Support (BLS) certification.
• Ongoing education related to acute stroke care.
• Adherence to regulatory standards.
