Physical Therapist – Outpatient Rehabilitation

About the role

Vanderbilt Health is seeking a Physical Therapist to join the growing rehabilitation team at the Dayani Center for Health & Wellness. The Dayani Center is a specialized outpatient program recognized for its interdisciplinary approach to oncology rehabilitation, prehabilitation, chronic pain management, and medically complex care.

Responsibilities

• Evaluates and treats patients experiencing functional limitations related to cancer, cancer treatments, chronic pain syndromes, fatigue, deconditioning, neuromuscular conditions, muscle tension dysphonia, and other complex medical diagnoses.
• Develops individualized treatment plans designed to restore function, improve quality of life, and help patients achieve their personal goals.
• Treatment interventions may include: therapeutic exercise and strengthening, functional mobility training, neuromuscular re-education, endurance and conditioning programs, cancer rehabilitation and prehabilitation, chronic pain management strategies, symptom management and energy conservation, aquatic therapy, functional and community reintegration activities.
• Collaborates with the healthcare team to achieve patient care, clinical outcome, operational, and system improvement objectives.
• Applies appropriate, evidence-based interventions to ensure effective outcomes and re-evaluates performance to assess patient's progress towards goals.
• Promotes successful return to home, community, and school/vocational environments.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Tennessee Physical Therapist license or license eligible.
• Preferred experience in outpatient rehabilitation, oncology rehabilitation, neurologic rehabilitation, chronic pain management, aquatic therapy, hospital-based rehabilitation, or related specialty areas.
• Strong clinical reasoning and communication skills.
• Commitment to patient-centered, evidence-based care.
• Passion for interdisciplinary teamwork and continuous learning.
