Physical Therapist, Outpatient Rehab, 40 hours, Danielson, CT

Job Summary

The Physical Therapist will organize and administer prescribed physical therapy programs to help patients achieve their maximum functional potential. Responsibilities include evaluating and treating patients, educating patients and significant others, and maintaining professional growth.

Responsibilities

• Evaluates and treats patients as prescribed by referring physician.
• Records treatment, response, progress and discharge information in patient's chart or enters information into computer.
• Educates patient, family and significant others regarding condition, treatment and home program.
• Routinely confers with physician and other practitioners in person, by phone, or in writing, regarding progress, regression or change of plan.
• Orients, instructs, and directs assistants, aides and students.
• Maintains professional growth by attending department meetings, inservice and educational conferences.
• Aids in the planning and implementation of new programs.

Requirements

• BS, MS or DPT degree from an accredited program in Physical Therapy required.
• Licensure by the state of Connecticut required.
• Current AHA Healthcare Provider CPR certification or equivalent within 3 months of hire required.
• Minimum of one-year clinical experience preferred.

Knowledge, Skills, and Abilities

• Ability to review physicians' referral, patients' condition, and medical records to determine physical therapy treatment required.
• Demonstrate skills in order to perform tests and measure patients' strength, motor development, sensory perception, functional capacity, along with respiratory and circulatory efficiency.
• Plan and prepare written treatment program based on evaluation of patient data with realistic, objective, long and short-term goals and time frames.
• Records treatment, response, progress and discharge information in patients' chart or enters information into computer, in measurable and objective terms as dictated by department policy.
• Knowledge to administer individualized programs of physical therapy as determined by the physicians' order and the initial evaluation. This will include, but not be limited to, therapeutic exercise, moist heat, traction, manual therapy, cold, gait training, hydrotherapy, ultrasound and electrical stimulation.

Schedule

Day Shift (can be flexible between hours of 7am-6pm). Options may include: Standard 5 × 8-hour shifts (8:00 a.m. – 4:30 p.m.), Early-in, early-out schedule (7:00 a.m. – 3:30 p.m.), Option for 4 × 10-hour shifts per week available for experienced staff.
