Physical Therapist - Outpatient Orthopedic

About the role

We are seeking a compassionate Physical Therapist (PT) to join an outpatient orthopedic team in Fond du Lac, WI.

Responsibilities

• Performing initial and ongoing assessments of patients' conditions
• Establishing, revising, and evaluating individualized treatment plans involving patients and families
• Applying evidence-based physical therapy techniques to treat movement dysfunction and pain
• Providing patient and staff education tailored to needs
• Ensuring continuity of care through discharge planning, home instructions, equipment recommendations, and follow-up care
• Overseeing activities of support personnel and maintaining detailed documentation of therapy responses

Qualifications

• Current licensure as a Physical Therapist in Wisconsin
• Strong clinical reasoning and communication skills
• Experience in outpatient or specialty care preferred
• Knowledge of safety protocols and ability to work alertly
