Physical Therapist - Outpatient Orthopedic

About the role

Join our outpatient orthopedic team as a full-time Physical Therapist (PT) in Fond du Lac, WI, providing compassionate, evidence-based, patient-centered care in a collaborative environment.

Responsibilities

• Perform initial and ongoing assessments of patients' conditions
• Develop, revise, and evaluate individualized care plans involving patients and families
• Utilize standard physical therapy techniques to treat movement dysfunction and pain
• Educate patients, families, and staff on treatment and care needs
• Ensure continuity of care through discharge planning, home instructions, and equipment recommendations
• Oversee activities of support personnel such as assistants and students
• Maintain accurate documentation of patient responses and interventions
• Work safely and perform additional duties as assigned

Requirements

• Current Physical Therapist licensure in Wisconsin
• Strong clinical reasoning and communication skills
• Passion for patient-centered care and team collaboration
• Experience in outpatient or specialty settings preferred
