Physical Therapist - Outpatient/Home Care Per Diem

About the Role

Provide and coordinate physical therapy programs to support rehabilitation, promoting independence, health, and overall well-being. Collaborate with the patient care team, ensuring respectful, culturally sensitive, and compassionate care.

Responsibilities

• Assess clients, develop individualized treatment plans, and implement therapy techniques for mobility, strength, balance, and sensory awareness.
• Design and fit adaptive devices, train clients in their use, and select appropriate physical modalities.
• Supervise support staff, ensure accurate documentation, and contribute to team meetings and training programs.
• Educate patients and families, fostering trust and understanding, while maintaining confidentiality and adhering to policies.
• Participate in quality improvement, safety protocols, and professional development activities.

Requirements

• Current Physical Therapy license.
• Proficiency in diagnostic assessment, treatment planning, and evidence-based therapy methods.
• Strong communication, teamwork, leadership, and cultural sensitivity skills.
• Commitment to safety practices and infection control.
