Physical Therapist - Outpatient/Home - Per Diem

Position Overview

Organize and perform physical therapy programs to aid rehabilitation, focusing on restoring and enhancing client performance and promoting independence. Collaborate with the patient care team, respecting patient dignity and cultural differences, to deliver safe, high-quality care.

Responsibilities

• Assess patients, develop individualized treatment plans, and implement evidence-based therapies targeting mobility, strength, balance, and sensory awareness.
• Design and fit adaptive devices, instruct clients on their use, and select appropriate physical modalities.
• Supervise supportive personnel, ensure accurate documentation, and contribute to team meetings.
• Educate patients and families, promote a compassionate environment, and participate in training programs for staff and medical personnel.
• Adhere to safety protocols, maintain confidentiality, and engage in quality improvement initiatives.

Qualifications & Skills

• Current Physical Therapy MA Licensure.
• Strong communication, leadership, and problem-solving abilities.
• Proficiency in electronic medical record documentation.
• Physical capacity to perform patient transfers, stand/walk extensively, and handle physical demands of the role.
• Commitment to patient-centered, culturally sensitive care.

This role is on-site, focusing on delivering comprehensive physical therapy services to support patient rehabilitation and well-being.
