Physical Therapist | Outpatient

About the Role

Responsible for coordinating and delivering physical therapy services to patients, implementing individualized treatment programs, and promoting health and recovery within various settings.

Responsibilities

• Evaluate patients through records, interviews, observations, and team input.
• Develop, implement, and modify personalized treatment plans based on ongoing assessments.
• Provide direct therapy services aligned with patients' physical, emotional, and intellectual needs.
• Document evaluations, treatment plans, progress notes, and reports accurately.
• Communicate effectively with patients, families, physicians, and interdisciplinary team members.
• Build rapport and collaborate with team members throughout patient care.
• Participate in educational activities for professional growth.

Requirements

• Active physical therapy license in the practice state.
• Visual acuity and effective communication skills.
• Preferred 1-3 years of licensed physical therapy experience.
• Commitment to patient-centered care, confidentiality, and team collaboration.
• Adherence to safety standards and professional standards set by regulatory bodies.
