About the Role
This individual works collaboratively within a multidisciplinary team to develop an individualized physical therapy treatment plan for clients in their home or residence at New Hampton Medical Center. The position is full-time (40 hours weekly) on a day shift (8-hour shift).
Responsibilities
- Evaluates clients to obtain and interpret data necessary for treatment planning and implementation in the client's home setting.
- Performs initial assessment/start of care services, including OASIS assessment for Physical Therapy (PT) clients when applicable.
- Develops an individualized plan of care involving the client, specific to the needs identified while collaborating with the physician.
- Communicates with the physician regarding any requests to revise the plan of care.
- Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.
Qualifications
- Graduate of a university approved by the American Physical Therapy Association.
- Licensed in the state of Iowa to practice physical therapy.
- Minimum of 1-year experience preferred.
- Community or home health experience desirable.
