Physical Therapist

About the Role

This individual works collaboratively within a multidisciplinary team to develop an individualized physical therapy treatment plan for clients in their home or residence at New Hampton Medical Center. The position is full-time (40 hours weekly) on a day shift (8-hour shift).

Responsibilities

• Evaluates clients to obtain and interpret data necessary for treatment planning and implementation in the client's home setting.
• Performs initial assessment/start of care services, including OASIS assessment for Physical Therapy (PT) clients when applicable.
• Develops an individualized plan of care involving the client, specific to the needs identified while collaborating with the physician.
• Communicates with the physician regarding any requests to revise the plan of care.
• Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.

Qualifications

• Graduate of a university approved by the American Physical Therapy Association.
• Licensed in the state of Iowa to practice physical therapy.
• Minimum of 1-year experience preferred.
• Community or home health experience desirable.
