Physical Therapist or PTA Home Care

We are looking for a compassionate, reliable PT and PTA to join our team to provide excellent care and who enjoys working with patients in an at-home setting. Our mission is to help individuals maintain their independence and improve their quality of life—all from the comfort of their homes.

Responsibilities

• Provides care and services in accordance with the plan of care.
• Assists community members and health field personnel to assess, plan for, and provide needed health and related services.
• Documents care provided in an accurate and timely manner.
• Provides teaching to clients and families.
• Coordinates and collaborates with other members of the health care team, community agencies, and referral sources.
• Communicates the plan of care with patients and families and ensures understanding of the exercise program prescribed.
• Responsive to meeting or exceeding patient/client expectations and promoting a positive image of the company.
• Adheres to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.
• Attends continuing education programs.
• Other duties assigned by the department manager.

Requirements

• Associate's degree and graduation from an accredited Physical Therapy program.
• One year of physical therapy experience preferred.
• Possesses and maintains CPR Certification.
• Possess valid driver's license.
• Must be licensed by the State of Massachusetts as a Physical Therapy Assistant.

Schedule

• Part-time or per diem.
• 10 – 30 hours per week.
• Monday to Friday.
