We are looking for a compassionate, reliable PT and PTA to join our team to provide excellent care and who enjoys working with patients in an at-home setting. Our mission is to help individuals maintain their independence and improve their quality of life—all from the comfort of their homes.
Responsibilities
- Provides care and services in accordance with the plan of care.
- Assists community members and health field personnel to assess, plan for, and provide needed health and related services.
- Documents care provided in an accurate and timely manner.
- Provides teaching to clients and families.
- Coordinates and collaborates with other members of the health care team, community agencies, and referral sources.
- Communicates the plan of care with patients and families and ensures understanding of the exercise program prescribed.
- Responsive to meeting or exceeding patient/client expectations and promoting a positive image of the company.
- Adheres to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.
- Attends continuing education programs.
- Other duties assigned by the department manager.
Requirements
- Associate's degree and graduation from an accredited Physical Therapy program.
- One year of physical therapy experience preferred.
- Possesses and maintains CPR Certification.
- Possess valid driver's license.
- Must be licensed by the State of Massachusetts as a Physical Therapy Assistant.

- The ability to work in a team-oriented environment as well as independently.
Schedule
- Part-time or per diem.
- 10 – 30 hours per week.
- Monday to Friday.
