Job Description

Provides skilled physical therapy services to patients in their homes, focusing on improving mobility, strength, balance, safety, and independence.

  • Conduct in-home PT evaluations and reassessments
  • Create and progress individualized treatment plans
  • Provide therapeutic exercises, gait training, balance training, and functional mobility training
  • Educate patients and caregivers on home exercise programs and safety strategies
  • Document visits and patient progress according to clinical standards
  • Collaborate with healthcare teams to support functional recovery

Licensed Physical Therapist who is comfortable working independently, managing a mobile caseload, and helping patients achieve safe functional outcomes in their home environment.
