Physical Therapist- Home Health Visits

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per‑diem role performing home health visits for senior living clients throughout Monroe County. The PT will join a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain safely at home.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents.
• Develop and implement individualized care plans that incorporate manual therapeutic exercises, gait training, balance work, and other interventions.
• Continually assess and revise the PT care plan and collaborate with nursing on the multidisciplinary care plan as appropriate.
• Educate and instruct clients, family members, or other representatives on rehabilitative care and activities that promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.

Qualifications

• Current Pennsylvania Physical Therapist license.
• Minimum of one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Pay

Base Pay, depending upon qualifications and subject to negotiation: $65.00 – $75.00 per point.
