Physical Therapist- Home Health Visits

BAYADA Home Health Care is seeking an experienced Physical Therapist for Full Time, Part Time, or Per Diem home health visits throughout South and West Philadelphia. This role serves adult and geriatric clients on a per-visit basis.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, including mobility/range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the Physical Therapy care plan and participate in multidisciplinary care planning with nursing as appropriate.
• Educate and instruct clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touchpad tablet.

Requirements

• A current Pennsylvania Physical Therapist license.
• A minimum of one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association (verified by school transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Pay

Base pay ranges from $65.00 to $75.00 per point, depending on qualifications and subject to negotiation.
