About the role

Careage Home Health is seeking a Physical Therapist to join our team. This position offers the opportunity to grow your PT skillset while providing essential one‑on‑one care to individuals in the home care setting. You will travel to multiple locations in the Greater Seattle Area and work full time with a flexible schedule (no on‑call, no weekends). The role includes a competitive hourly rate for patient visits, documentation/charting, and travel time, ranging from $53‑73 per hour DOE.

Responsibilities

• Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
• Work in conjunction with all other home care personnel in patient care planning.
• Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
• Take initial patient history and make initial evaluation.
• Consult with physicians regarding change in treatment.
• Instruct patients and family in home programs and activities of daily living.
• Participate in Quality Assessment and Performance Improvement activities as assigned.
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
• Appropriately utilize ICD‑10 codes.

Requirements

• Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
• Currently licensed as a physical therapist in the state of Washington.
• Two (2) years’ experience as a physical therapist (preferred).
• Home Health experience (preferred).
• Detail oriented, self‑motivated and capable of efficient time management.
• Critical thinker capable of working collaboratively with a team.
• Valid WA State driver’s license.
