About the role
Assess and evaluate patient needs related to mobility, endurance, pain, and medical conditions in a home health setting.
Responsibilities
- Evaluate patients' functional status and assist in developing personalized therapy plans.
- Provide therapy services within state regulations and collaborate with the healthcare team.
- Monitor and report patient responses and progress.
- Implement treatment interventions to improve or maintain patient function and relieve pain.
- Educate patients, families, and care team members about care plans and progress.
- Complete patient visits, documentation, and evaluations per policy and guidelines.
Requirements
- Current state licensure as a Physical Therapist.
- CPR certification, valid driver’s license, and reliable transportation.
- Ability to serve patients regardless of background or circumstances.
