Physical Therapist (Home Health)

Role Overview

Provides specialized physical therapy services to restore and enhance patients' physical, social, and mental functions within a home health or hospice setting. Acts as a co-leader in interdisciplinary teams to develop and implement personalized treatment programs, ensuring quality care and achieving departmental goals.

Key Responsibilities

• Conducts comprehensive evaluations and develops individualized treatment and discharge plans.
• Administers therapy interventions using approved protocols and techniques.
• Ensures timely and accurate clinical documentation of patient care.
• Supervises assigned therapy assistants and collaborates with healthcare team members.
• Performs additional duties related to patient care and program support.

Qualifications & Skills

• Doctor of Physical Therapy (DPT) degree or equivalent.
• Valid state licensure in physical therapy.
• CPR/BLS certification from an approved provider.
• Valid driver's license and minimum age of 21.

Organizational Requirements

• Adheres to vaccination policies, with allowances for medical or religious exemptions.
• Participation in E-Verify is required, confirming eligibility to work.
