About the Role
Careage Home Health is seeking a Physical Therapist to join our team. This position offers a great opportunity to grow your PT skillset while providing essential one-on-one care to individuals in the home care setting. The role involves traveling to multiple locations in the Renton area.
Responsibilities
- Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
- Work in conjunction with all other home care personnel in patient care planning.
- Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
- Take initial patient history and make initial evaluation.
- Consult with physicians regarding change in treatment.
- Instruct patients and family in home programs and activities of daily living.
- Participate in Quality Assessment and Performance Improvement activities as assigned.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition.
- Appropriately utilize ICD-10 codes.
Requirements
- Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
- Currently licensed as a physical therapist in the state of Washington.
- Two (2) years' experience as a physical therapist, preferred.
- Home Health experience, preferred.
- Detail-oriented, self-motivated, and capable of efficient time management.
- Critical thinker capable of working collaboratively with a team.
- Valid WA State driver's license.
