About the role
Pride Health is actively hiring a Physical Therapist to join our client's Home Health Program in a growing care team serving patients.
Schedule
- Monday - Friday, 8:00 AM - 4:30 PM
- Duration: 6+ Months
- Weekly Hour Guarantee: 36 - 40 hours
- On-Call/Weekend Requirement: No
Pay
- Local Pay: $52.00 - $56.00/hour (W-2) Plus Mileage reimbursement
- Travel Pay Package: Up to $2,700/week gross (includes weekly non-taxable stipend) Plus Mileage reimbursement
- Pay offered is based on experience, expertise, credentialing, and education
- Mileage Reimbursement: Yes (from first patient to last patient)
Responsibilities
- Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks
- Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence
- Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education
- Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques
- Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems
- Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides
- Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team
- Participate in case conferences, care coordination activities, and team collaboration
- Ensure compliance with CMS regulations, infection control standards, and agency policies
- Manage a mobile caseload efficiently while meeting productivity and documentation expectations
- Adapt quickly to new environments, workflows, and patient populations as a travel clinician
Requirements
- Graduate of an accredited Physical Therapy program
- Active Physical Therapist license in the state of assignment
- Home Health experience required
- Willingness to travel within the assigned coverage area
- Current BLS certification
- Valid driver's license
