Pay

• Start of Care (SOC): $110.00
• Evaluation / Re-Admission: $80.00
• Routine Visit: $70.00
• Reliable bi-weekly electronic payments based on completed Kinnser visit logs—no invoice tracking required.

Territory

North Palm Beach County: Jupiter, Palm Beach Gardens, Riviera Beach, Juno Beach, and surrounding areas.

About the Role

Join a clinician-owned and operated home care team led by a Physical Therapist with 20 years of experience. We provide clinician-to-clinician support and value your expertise.

• Per-visit compensation with set flat rates to respect your time and travel.
• Efficient documentation using Kinnser (WellSky).

Responsibilities

• Provide high-quality, one-on-one physical therapy to geriatric patients in their homes.
• Implement effective plans of care to improve patient mobility and safety.
• Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge).
• Collaborate with clinical leadership, office staff, and therapy assistants to deliver outstanding patient care.

Requirements

• Active Florida Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid Driver’s License and reliable transportation.
• Kinnser/WellSky experience is a plus.
