Old Glory Home Health Care & Assistive Care is a clinician-owned, Medicare-certified, CHAP-accredited agency with Age-Friendly Care certification. We are seeking a Physical Therapist for part-time or PRN home health visits in communities east and northeast of Richmond, serving Sandston, Varina, Highland Springs, Mechanicsville, Manquin, Hopewell, and New Kent, Virginia. Responsibilities

• Evaluate patients and develop individualized physical therapy plans of care.
• Provide skilled treatment to improve strength, balance, mobility, and functional independence.
• Educate patients and caregivers on home exercises, fall prevention, and safe movement.
• Collaborate with physicians, nurses, and other rehabilitation professionals to coordinate care.
• Supervise physical therapist assistants as assigned.
• Complete accurate, timely documentation in our electronic medical record.

Qualifications

• Active Virginia Physical Therapist license.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently while collaborating with a multidisciplinary team.
• Reliable transportation for patient visits.
• Home health experience preferred.
