Homecare RNs needed

About the role

Alvita Care is seeking experienced RNs for clients throughout Westchester County. The position offers opportunities to make skilled nursing visits, with the ability to work full‑time or around your current schedule. Shifts are assigned to provide 1:1 care in the home environment.

Responsibilities

• Assess needs of the clients to develop a plan of care
• Consult with Management on complex/unique cases
• Provide a plan of care with support of the Clinical/Medical/appropriate multidisciplinary members to ensure services follow the plan of care through assessment, planning, coordination, counseling, teaching, and supervision
• Monitor/assess patients' condition and effects of the care plan; oversee/supervise care provided by Licensed Practical Nurses and Paraprofessionals
• Manage, plan, coordinate, and execute the patient's safe discharge from service
• Ensure physician's orders, discharge summary, and communication to team members, patient, and family adhere to company and regulatory requirements
• Communicate and consult with the patient's MD as needed; manage and coordinate all aspects of assigned caseload
• Assist patients with activities of daily living
• Document content and process of all services provided in the internal system properly
• Perform other related duties and participate in special projects as assigned

Qualifications

• NYS RN licensure
• BLS certification
• Minimum of one (1) year demonstrated experience as an RN
• Ability to put patients at ease and explain physician's instructions
• Ability to multi‑task with excellent organization skills
• Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds
• Knowledge of medical terminology
• Team player and ability to work independently

Schedule

Shifts range from 8 to 12 hours, with options for full‑time or flexible scheduling around your current work commitments.
