Homecare RNs needed

About the role

Alvita Care is seeking experienced Registered Nurses (RNs) for positions in Westchester County. Opportunities include shifts ranging from 8 hours to 12 hours, with the possibility of making skilled nursing visits.

Responsibilities

• Assess client needs to develop a care plan
• Consult with management on complex/unique cases
• Develop and implement a care plan with support from multidisciplinary members
• Monitor and assess patients' conditions
• Supervise the care provided by LPNs and paraprofessionals
• Ensure patient safe discharge from service
• Communicate with patients' doctors as needed
• Manage and coordinate all aspects of assigned caseload
• Document all services provided in the internal system
• Perform other related duties as assigned

Qualifications

• NYS RN licensure
• BLS certification
• Minimum of one (1) year demonstrated experience as an RN
• Ability to put patients at ease and explain physician's instructions
• Excellent organizational skills
• Experience working with diverse populations
• Knowledge of medical terminology
• Team player and ability to work independently
