Responsibilities

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of Physical Therapy.
• Provides the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages, and evaluates the provision of both professional and ancillary home health services (direct care and case management).
• Maintains the activities of the multi-disciplinary team providing care to the patient.
• Maintains oversight of the provision of therapeutic services through written and verbal communication with all disciplines.
• Develops, implements, and carries out a discharge plan in collaboration with the patient, family/caregiver, and members of the healthcare team.
• Interprets agency policies to patients/families.
• Reviews patients' visits weekly and provides a plan to the Patient Care Supervisor and team assistant, including anticipated weekend visits.
• Communicates any schedule changes and patient care issues to the Patient Care Supervisor.

Requirements

• BLS (Basic Life Support)
• PT (Physical Therapist) License
• Driver's License from the state of Florida (required within 30 days of hire)

Qualifications

• Bachelor's degree in Physical Therapy
• Prior experience in Physical Therapy, preferably 1 year
• One year of experience in home care preferred

Schedule

Shift hours are 8:00 AM - 5:00 PM, Monday through Friday. Occasional on-call work is available.
