PHYSICAL THERAPIST - HOME HEALTH

Responsibilities

• Conducting physical therapy assessments and delivering skilled treatments
• Cooking up client care and counseling patients and caregivers
• Documenting interdisciplinary communication and supervising ancillary staff
• Problem-solving, critical thinking, and ensuring safe patient transfers
• Developing and managing plans of care

Qualifications and Skills

• Minimum Bachelor's degree; Master’s or Doctorate preferred
• Ability to assist with patient lifts, transfers, and functional activity training
• Strong communication skills, confidentiality, crisis management, and customer service
• Proficiency with Electronic Health Records, Microsoft Office tools, and a valid driver’s license with minimal violations
• Current PT license and BLS certification
