Physical Therapist (Home Health)

Location: Brookings, OR | Full-Time, Onsite/Community-Based | Monday–Friday, 8:00 AM–5:00 PM | Home Health / Community-Based Patient Care (Patients' Homes)

About the role

We are seeking a compassionate Physical Therapist to provide home-based rehabilitation services, helping patients improve mobility, restore function, and maintain independence.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises, gait training, mobility training, and functional rehabilitation.
• Educate patients and caregivers on home exercise programs and safe mobility.
• Monitor patient progress and maintain accurate clinical documentation.
• Coordinate care with physicians and interdisciplinary healthcare teams.
• Supervise Physical Therapist Assistants as required.

Requirements

• Graduate of an accredited Physical Therapy program.
• Active, unrestricted Oregon Physical Therapist license.
• Current BLS certification or ability to obtain before starting.
• Valid driver's license, auto insurance, and reliable transportation.
• Strong communication, organizational, and clinical assessment skills.
• One year of experience preferred; new graduates are welcome.

Additional Details

• Approximately 2–6 home visits per day.
• Average daily travel of 25–50 miles.
• No regular on-call responsibilities.
• Primarily serves geriatric and other home health patients.
• Anticipated hiring timeline: Early 2027.
